Until Change Healthcare fully recovers from the cyber-attack, we will be sending statements directly from Midwest Health Partners. These statements will look different than what you are used to, but be assured they are coming from Midwest Health Partners by looking at the remittance address. The address should be PO Box 209, Norfolk, NE 68702-0209. Payments can also be made by calling 402.379.2322 or you can stop by the billing office at 1410 N. 13th Street, Norfolk, NE.
